Verify Claims Status-
Find date of service and you can see how much was paid, when check was issued,
the transaction number as well as the check number so you can track as needed.

Eligibility- Enter
Employee SS# to view currently covered family members.

Plan Information -
Enter Employee 4 digit plan number to get access to all their group plan
information including description of plan.
